But not everyone benefits from daily aspirin therapy.<\/p>\n<p><strong>What is Aspirin?<\/strong><\/p>\n<p>As early as Hippocrates (400 BC), people were harvesting willow to treat pain. Today, <a href=\"http:\/\/www.aspirin-foundation.com\/what\/100.html\" target=\"_blank\">aspirin is<\/a>: &#8220;<em>Acetyl salicylic acid . . . a synthetic derivative of a compound, salicin, which occurs naturally in plants, notably the willow tree.&#8221;<\/em><\/p>\n<p>When derived straight from salicin, salicylic acid is &#8220;<a href=\"http:\/\/www.aspirin-foundation.com\/what\/100.html\" target=\"_blank\">hard to swallow, causing irritation to the lining of the mouth and stomach<\/a>.&#8221; In response, a buffer was added to make the medicine easier to take and a &#8220;process of synthesizing the acetyl salicylic acid&#8221; was developed.<\/p>\n<p><strong>How Does it Work, Generally?<\/strong><\/p>\n<p>To reduce pain and inflammation, <a href=\"http:\/\/www.washingtonpost.com\/wp-dyn\/articles\/A8946-2004Nov23.html\" target=\"_blank\">aspirin blocks<\/a>: &#8220;<em>An enzyme known as prostaglandin synthetase or cyclooxygenase. The body needs this enzyme to make precursors for most of the half-dozen known prostaglandins [(hormone-like substances) including] . . . those that make it easier for nerve cells to pass pain signals from one to another, those that raise fever and those that promote the swelling of inflamed tissue.&#8221;<\/em><\/p>\n<p><strong>How Does Aspirin Prevent Heart Attacks and Strokes?<\/strong><\/p>\n<p>When you suffer a cut or other bleeding injury, platelets (clotting cells in the blood) rush to the wound and build up, forming a seal that stops the vessel from bleeding. In order to be available to perform this life-saving function at a moment&#8217;s notice, platelets must be present throughout the circulatory system. Although in a perfectly healthy blood vessel platelets pose no threat, when a person suffers from <a href=\"http:\/\/www.heart.org\/HEARTORG\/Conditions\/Cholesterol\/WhyCholesterolMatters\/Atherosclerosis_UCM_305564_Article.jsp\" target=\"_blank\">atherosclerosis<\/a> (narrowing or hardening of the arteries due to build-up of substances including cholesterol), the platelets could add to the build-up and form a blockage \u2013 which leads to a heart attack or stroke.<\/p>\n<p>Luckily, the <a href=\"https:\/\/www.careinternet.net\/caregiver\/aspirin.php\" target=\"_blank\">thromboxane that activates and aggregates platelets<\/a> is as susceptible to the anti-inflammatory effects of aspirin as the prostaglandins that cause fever, pain and swelling: &#8220;<em>The drug blocks a platelet enzyme, cyclo-oxygenase, by acetylating the enzyme&#8217;s active site. Inhibition of the enzyme blocks production of an important prothrombotic agent known as thromboxane A2 . . .&#8221;<\/em><\/p>\n<p>Or in <a href=\"http:\/\/weill.cornell.edu\/cert\/patients\/preventing_heart_attacks_and_strokes_with_aspirin.html\">plain language<\/a>: &#8220;<em>Aspirin prevents heart attacks and strokes by permanently blocking one path to a platelet&#8217;s ability to become &#8220;activated&#8221; and cause a clump of platelets in a blood vessel . . . [which] blocks blood flow which causes the attack.&#8221;<\/em><\/p>\n<p>Of course, even on aspirin therapy, your blood will still clot \u2013 just not as efficiently. It would be counterproductive to take a medicine that made you bleed to death.<\/p>\n<p><strong>Why Isn&#8217;t Everyone Taking Daily Aspirin Therapy?<\/strong><\/p>\n<p>Since it <a href=\"http:\/\/weill.cornell.edu\/cert\/patients\/preventing_heart_attacks_and_strokes_with_aspirin.html\" target=\"_blank\">decreases the platelets&#8217; ability to clot<\/a>: &#8220;<em>We do bleed more when taking aspirin. Also, aspirin is irritating to the stomach, so it can cause ulcers which then bleed a lot since the platelets don&#8217;t clump as well to stop the bleeding.&#8221;<\/em><\/p>\n<p>Adding insult to injury, &#8220;<a href=\"http:\/\/www.health.harvard.edu\/press_releases\/daily-low-dose-aspirin-can-prevent-heart-attack-and-stroke-but-is-often-misused\" target=\"_blank\">aspirin inhibits the formation of substances that protect the stomach&#8217;s delicate lining<\/a>,&#8221; a condition which contributes to even more bleeding in the gut. Furthermore, in a recent Women&#8217;s Health Study, it was show that aspirin therapy was &#8220;<a href=\"http:\/\/www.health.harvard.edu\/newsweek\/Aspirin_and_women_age_matters.htm\" target=\"_blank\">linked with an increased risk of bleeding (hemorrhagic) strokes<\/a>&#8221; in women.<\/p>\n<p><strong>So Who Should Take Aspirin Therapy?<\/strong><\/p>\n<p><strong>Generally<\/strong><\/p>\n<p>Recent &#8220;<a href=\"http:\/\/www.health.harvard.edu\/press_releases\/daily-low-dose-aspirin-can-prevent-heart-attack-and-stroke-but-is-often-misused\" target=\"_blank\">clinical trials seem to show that aspirin may not prevent first [heart] attacks<\/a>&#8221; or strokes, although experts caution people from reading too much into this. First, they warn it is difficult to conduct research on the efficacy of a treatment in people who have never suffered from the condition, and second, aspirin works differently depending on the type of delivery (coated, sustained release, etc.). So, the findings may not be as definitive as they might seem upon first reading.<\/p>\n<p>After considering the costs and benefits of daily aspirin therapy, at least <a href=\"http:\/\/www.health.harvard.edu\/press_releases\/daily-low-dose-aspirin-can-prevent-heart-attack-and-stroke-but-is-often-misused\" target=\"_blank\">one doctor has concluded<\/a>:<\/p>\n<blockquote><p><em>Aspirin is likely to be beneficial in preventing strokes and heart attacks in people at relatively high risk for these events [those with a family history of death by heart attack at age younger than 60], and likely to be harmful by causing excessive bleeding in people at low risk for these events [those whose parents lived to be at least 80].<\/em><\/p><\/blockquote>\n<p><strong>Women Wrinkle<\/strong><\/p>\n<p>In the <a href=\"http:\/\/www.health.harvard.edu\/newsweek\/Aspirin_and_women_age_matters.htm\" target=\"_blank\">10-year Women&#8217;s Health Study<\/a> of 40,000 female health workers age 45 or older, it was revealed that, overall: &#8220;<em>Aspirin had no effect on heart attacks, nor did it influence deaths from heart disease or stroke.&#8221;<\/em><\/p>\n<p>The researchers did, however, find that it <a href=\"http:\/\/www.health.harvard.edu\/newsweek\/Aspirin_and_women_age_matters.htm\" target=\"_blank\">reduced the risk of stroke by 17%<\/a>, a fact deemed: &#8220;<em>Important . . . since more women have strokes each year than heart attacks. In 2007, <a href=\"http:\/\/www.businessweek.com\/magazine\/content\/11_27\/b4235023437718.htm\" target=\"_blank\">Bayer aspirin boasted sales of $1.1 billion<\/a>.<\/li>\n<li>Each year, over <a href=\"http:\/\/www.cdc.gov\/heartdisease\/facts.htm\" target=\"_blank\">700,000 Americans suffer a heart attack<\/a>, and over 25% of them have already had at least one heart attack previously.
